STEP 1
Download, print out the form(s) below, and fill it out.
- *You can type in information on the PDF form except Name (Account Holder), which must be handwritten by yourself.
STEP 2
Send us the completed form by postal mail together with a copy of your identification verification documents listed on the reverse side of the "Notification of Individual/Corporate Number" form.
Use Address Label below when sending the form to us.
- When you put a stamp on an envelope to send us the form instead of using Address Label

STEP 3
Once we receive the form from you, it will take approximately 1 to 2 weeks to complete the process.
Once your My Number notification is complete, your My Number status becomes Notified in Registration InformationRegistration Information.

If you are advised over the phone that you are required to submit a designated form, download and print out the Cardholder's Dispute Form from the link below and fill it out.
- *The form is a "read-only" file. Please print out the form, fill out the required fields and send it to us by postal mail.
