SEAL/SIGNATURE CARD

  • *If you have not registered your SEAL/SIGNATURE within 3 months since you opened the account, please provide one of the identifications on SAMPLE, showing your full name and current address registered with us.

Fund Transfer and Remittance Payee Registration

  • *We only accept below fund transfer requests by postal mail.
    • Fund transfer whose amount is equivalent to 10 million yen or more.
    • Domestic remittance to other banks in foreign currency.
    • Foreign exchange yen clearing.
    • Fund transfer due to the account closure.

    Please take a photocopy of your application form before you send the form to us, because a customer copy of it will not be provided by the bank.
    Kindly confirm the amount of the remittance on your statement at "account details and activities" of online banking.

    Please return this form with the application for Cross Border Transfer. To make a fund transfer, the application for Cross Border Transfer and Notice on Transactions including Overseas Transfers and Non-Japanese Yen related Transactions are required.

    Request Forms are read-only. Please print the form, fill it out completely, and mail it to the address below. Request forms can be accepted by postal mail only. We do not accept these forms at branches, by facsimile or e-mail.
  • *Please refer to "Overseas Fund Transfer (Electronic transfers)" for details on how to make a fund transfer to abroad.
  • *A customer will be asked to notify his or her "My Number (Individual Number/Corporate Number)" when the customer initially transfers or receives funds to/from overseas via SMBC Trust Bank PRESTIA account opened on or after January 1, 2016. Please click here for details.

Address Change

  1. 1For a customer who does not have a mutual fund account with us, please download "Address Change Request Form/Residential Status Change Request Form", and complete the form and submit it to us by postal mail. A customer who would change his or her jurisdiction of residence may be asked to submit Self Certification under the "Common Reporting Standard" (Voluntary Notification or Notification of Change). In compliance with U.S. tax laws (including FATCA), additional document ("FATCA Certification Form" or "Substitute Form W-9") may be required if the change of address involves moving to/from the United States. Name Change

Kana Name Registration

"Kana (Katakana)" is a Japanese character, often used for domestic fund transfers.
If you do not register your kana name, your fund transfer may be returned or delayed. If you wish to additionally register your kana name, please fill out the form and mail it to us.

Switching your Banking Card to GLOBAL PASS

Notice

  • Cash Cards other than GLOBAL PASS are no longer available overseas from October 2021.
  • Please note that the following cards will be disabled automatically approximately two months after the GLOBAL PASS application is submitted to our bank.
    • Yen Savings Account Banking Card from which you switch to GLOBAL PASS
    • Supplementary Cards tied to the Yen Savings Account above
  • Debit PIN shall be the same PIN as your current Yen Savings Account Cash Card PIN for both Account Holder Card and Family Card. Please apply for GLOBAL PASS via Online Banking or contact PRESTIA Phone Banking and request the specific form if you wish to
    • Set Debit PIN different from your current Cash Card PIN.
    • Set different Debit PINs for yourself and Family Card holder(s).
  • If you do not remember your current Cash Card PIN, you cannot use this form. Please contact PRESTIA Phone Banking for further details.
  • If you need to change your registered address, please download Address Change Request Form, fill it out and send it back to us by postal mail along with GLOBAL PASS (Multi Currencies Visa Debit with Cash Card) Application (for switching) using Address Label.

Register or update "Status of Residence" and "Period of Stay (Date of Expiration)"

Please read Confirmation of Your "Status of Residence" and "Period of Stay", download and fill out "Change Request (Others)", and mail it to us with copies (front and back) of your Residence Card using Address Label.
